Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Perform home visit Physical Therapy assessments, delivering direct patient care to evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficient in EMR documentation.
• CPR Certification.
