Paradigm Rehab is currently seeking a dedicated Physical Therapy Specialist to join our compassionate team. As a Physical Therapist, you will have the rewarding opportunity to help individuals improve their quality of life by providing direct patient care and assessing medical conditions for potential rehabilitation.

Responsibilities

• Conduct home visit Physical Therapy assessments for referred patients.
• Evaluate functional capabilities, limitations, and potential for rehabilitation.
• Document patient care using EMR systems.
• Maintain a CPR Certification.

Requirements

• Graduated from an accredited Physical Therapy program.
• Valid New York Physical Therapy license.
• Experience in EMR documentation is a plus.
• Excellent communication and interpersonal skills.
