Responsibilities

• Conduct home visit assessments to evaluate patients' needs.

• Document patient information accurately using EMR systems.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license in New York.
• Proficient in conducting assessments and developing treatment plans.
• CPR certification.
