Job Description

Responsibilities
- Deliver and coordinate quality patient care in compliance with physician orders.
- Observe and assess patient conditions, report changes, and update care plans.
- Document all activities and care delivery in a timely manner.
- Participate in the implementation and updating of nursing care plans.
- Educate patients and families on disease processes and self-care techniques.
Requirements
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as an LPN.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance.
- Specific state requirements may apply based on practice location.
Preferences
- Recent experience as an LPN in a clinical care setting.
- Home health experience is a plus.
