Responsibilities

• Deliver and coordinate quality patient care in compliance with physician orders.
• Observe and assess patient conditions, report changes, and update care plans.
• Document all activities and care delivery in a timely manner.
• Participate in the implementation and updating of nursing care plans.
• Educate patients and families on disease processes and self-care techniques.

Requirements

• Graduate of an accredited school of nursing.
• Current, unrestricted state license as an LPN.
• Current CPR certification.
• Proficiency in clinical assessments, documentation, and compliance.
• Specific state requirements may apply based on practice location.

Preferences

• Recent experience as an LPN in a clinical care setting.
• Home health experience is a plus.
