Responsibilities

• Deliver quality patient care in accordance with physician orders.
• Assess patient condition and report changes to healthcare team.

• Implement and update nursing care plans.
• Educate patients and families on self-care techniques.

Requirements

• Graduate of an accredited nursing school.
• Current, unrestricted state LPN license.
• CPR certification.
• Proficiency in clinical assessments and documentation.

Preferences

• 6 months of recent clinical care experience.
• Home health experience.

Physical Requirements
