Essential Job Functions

  • Deliver and coordinate patient care in accordance with care plans and physician orders.
  • Observe and assess patient condition, reporting changes promptly.
  • Document all care activities and nursing actions accurately and in a timely manner.
  • Implement and update nursing care plans as needed.
  • Take necessary nursing actions to achieve expected outcomes and prevent risks.
  • Comply with Aveanna policies, regulations, and quality standards.
  • Provide patient education on self-care, disease processes, and preventive measures.
  • Participate in educational programs and skill evaluations to maintain competency.

Requirements

  • Graduate of an accredited nursing school.
  • Current, unrestricted LPN license in the state of practice.
  • Valid CPR certification.
  • Proficiency in clinical assessments, documentation, and nursing care compliance.

Preferences

  • Six months of recent clinical experience as an LPN.
  • Home health care experience.
