We are looking for an experienced Registered Nurse (RN) Field Nurse for Home Care to join our team. Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Develop and implement goal-oriented and patient-centered care plans.
• Ensure IV proficiency and perform IV functions.
• Develop discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain a BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of HomeHealth RN experience within the last 12 months.
• Previous Home Healthcare and Med-Surgical experience preferred.
