Job Description
We are seeking a dedicated Registered Nurse (RN) to join our Home Care team at ChristianaCare. As a Field Nurse, you will provide skilled nursing care and treatment to geriatric patients in the community, ensuring goal-oriented and patient-centered care plans.
Responsibilities
- Administer skilled nursing care and treatments to geriatric patients in the community.
- Develop and implement patient-centered care plans consistent with available resources.
- Prepare patients for discharge and provide necessary education.
- Collaborate with the healthcare team to ensure comprehensive patient care.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- Minimum 2 years of recent HomeHealth RN experience.
- IV proficient or willing to train for IV functions.
