Job Description
We are currently looking for a passionate and experienced Registered Nurse (RN) Field Nurse to join our Home Care team at ChristianaCare. As part of our team, you will have the opportunity to work with a state-of-the-art Electronic Health Record system and receive comprehensive OASIS-E training to excel in your Home Health career.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Ensure IV proficiency and provide IV functions as needed.
- Participate in discharge planning for patients.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of hire.
- Current DE RN license or compact state license.
- Minimum 2 years of recent Home Health RN experience.
- Prior experience in Home Healthcare and Med-Surgical is preferred.
