ChristianaCare HomeHealth is seeking a Full-Time Registered Nurse (RN) to join our dedicated team. As an RN Field Nurse in Home Care, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure quality patient outcomes. Responsibilities

• Deliver skilled nursing care and prescribed treatments to geriatric patients.
• Develop and implement goal-oriented, patient-centered care plans.
• Ensure IV proficiency and deliver high-quality care in alignment with best practices.
• Plan for patient discharge and continuity of care.

Requirements

• BSN required; ADN candidates must obtain BSN within 3 years of employment.
• Active DE RN license or compact state license.
• Minimum 2 years of recent HomeHealth RN experience.
• Experience in Home Healthcare and Med-Surgical is preferred.
• Proficiency in IV administration or willingness to train.
