Job Summary
The Registered Nurse (RN) will serve as a case manager for a group of patients, overseeing post-acute services in home settings, providing direct patient care, and coordinating with a multi-disciplinary team to ensure high-quality outcomes.
Essential Functions
- Manage assigned patient caseload
- Collect and analyze patient data
- Develop and oversee care plans
- Supervise clinical team and perform treatments as needed
- Evaluate patient progress
Requirements
- Graduate from an accredited School of Nursing
- BSN preferred
- Current RN license
- BLS certification required
- Valid driver's license
