Job Description
We are seeking a dedicated Registered Nurse (RN) specializing in Home Health to join our team at Atrium Health in Charlotte, North Carolina. In this role, you will provide expert care to adult and geriatric patients in their homes, ensuring they receive world-class healthcare and compassionate services.
Responsibilities
- Serve as a case manager overseeing post-acute services in the home setting.
- Provide direct patient care and supervise LPNs and HHAs to meet patient needs and individual goals.
- Coordinate care with a multidisciplinary team and act as the patient's liaison with healthcare providers.
- Ensure timely and qualified patient care and maintain proper documentation for optimal outcomes.
Requirements
- Graduate from an accredited School of Nursing; BSN preferred.
- Current RN license in the state of practice.
- BLS certification required.
- Valid driver's license.
