Responsibilities

• Conduct home visit Physical Therapy assessments for referred patients.
• Provide direct patient care to assess medical conditions, functional capabilities, and potential for rehabilitation.
• Document patient records using EMR systems.

Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation preferred.
• CPR Certification required.
