Responsibilities

• Conduct home visit assessments of referred patients to evaluate medical conditions and functional capabilities.
• Provide direct patient care, assess limitations, and develop rehabilitation plans.
• Utilize EMR documentation systems for accurate record-keeping.

Requirements

• Graduate of an accredited Physical Therapy (PT) program.
• Valid and unencumbered Physical Therapy (PT) license for New York.
• Proficiency in EMR documentation; experience with Hello Note is advantageous.
• CPR Certification.
