Responsibilities

• Perform home visit Physical Therapy assessments on referred patients to evaluate medical conditions and assess rehabilitation potential.
• Utilize EMR documentation for accurate record-keeping.
• Provide direct patient care to improve functional capabilities and quality of life.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficient in conducting physical therapy assessments and treatments.
• CPR Certification.
