Rehabilitation Therapist Responsibilities:

• Conduct home visit assessments of referred patients to provide direct patient care.
• Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation.

Rehabilitation Therapist Requirements:

• Graduated from an accredited Physical Therapy college or university.
• Unencumbered Physical Therapy license for New York.
• CPR Certification.
