Responsibilities

• Conduct home visit assessments of patients referred for therapy.
• Deliver direct patient care to assess medical conditions and rehabilitation potential.
• Document patient information accurately using our EMR system.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license in New York.
• Proficient in EMR documentation.
• Current CPR Certification.
