Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license in New York.
• Conduct home visit assessments and provide direct patient care.
• Proficient in EMR documentation; experience with Hello Note is a plus.
• Current CPR Certification.
