• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to assess medical conditions, functional capabilities, and potential for rehabilitation.
• Proficient in EMR documentation.
• CPR Certification.
