Genesis HealthCare, a leading provider of healthcare services, is seeking an LPN MDS Coordinator to join the team. As a crucial part of the Resident Assessment Process, the LPN MDS Coordinator ensures appropriate patient care and nursing center reimbursement through the completion of MDS items and CAAs.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to the interdisciplinary team.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare-related facility.
• Proficiency in general nursing practices, MDS completion, and Care Plans.
• Computer skills with experience in AccuMed preferred.
• Knowledge of RAI/PPS processes and BLS/CPR certification.
